Summary
A process is required to cover the situation in which an instruction failure occurs between an SMRA and a Data Aggregator and, although the SMRA may be informed of the fact, it is not able to resolve the failure. In these situations, issues may be left outstanding for long periods of time and the SMRAs do not have any procedures in place to deal with them.
